  • The cost for the OMMA card from the state $104.30, this should be in the form of debit or credit only.

  • If a patient has Medicare, Medicaid, or Sooner Care they will be charged only $22.50 for the OMMA Card

  • Patients will need to have the $104.30/$22.50 on a credit card or debit card
     

This drive is for Oklahoma residents only. 

For Arkansas patients: if they would like to get a temporary (30-day card) Oklahoma Medical Card we can assist with that. There is a fee of $104.30 for OMMA. (No breaks for Medicare, Medicaid, or Sooner Care) we do charge a $20.00 cash fee for submission.
